October 24, 2016 — October 30, 2016You can make your elections on the Your Benefits Resources™ (YBR) website beginning Monday, October 24, 2016 at 9:00 a.m., Eastern Time (ET), through Sunday, October 30, 2016. During this time, you may view your 2017 coverage options and costs, as well as enroll in or make changes to your 2017 default coverage online only. Click here.

You cannot call the Nokia Benefits Resource Center to enroll in or make changes to your 2017 coverage, or to ask questions about your 2017 plan options and pricing, until the online and phone enrollment period begins on Monday, October 31, 2016 at 9:00 a.m., ET.
ONLINE AND PHONE ENROLLMENT
October 31, 2016 — November 11, 2016The online and phone enrollment period begins Monday, October 31, 2016 at 9:00 a.m., ET, and ends Friday, November 11, 2016 at 5:00 p.m., ET. During this time, you can enroll in and/or change your 2017 Nokia health and welfare benefits coverage elections.
You must take action before Friday, November 11, 2016 at 5:00 p.m., ET. Late enrollments will not be accepted.

In mid-October, look for an email at your work address from “Nokia US Benefits” with the subject line: “2017 US Benefits: The Annual Open Enrollment Period Starts October 24, 2016.” Be sure to review this important information about your 2017 benefits and the annual open enrollment period.
YOU WILL NEED TO CREATE A USER ID
AND PASSWORD TO ENROLL

To access your personalized benefits information and to enroll on the YBR website, you will need to create a User ID and password. Just go to the YBR website anytime during your annual open enrollment period and follow the prompts to create them. If you forget one or both later on, you can request new one(s) through the YBR website. Your new User ID and password can be sent to you by email (if you added your email address to the YBR website when you first logged on) or by US mail. It may take up to 10 days to receive them through the mail.

COVERAGE (SBC) AVAILABLE ONLINE ON OCTOBER 24, 2016
SBCs for the health plans you are eligible for will be available on the YBR website beginning on October 24, 2016. You can use them to compare health plans when making decisions and enrolling in coverage.
