New for 2015: Tax Form 1095

As required by the Affordable Care Act (ACA) or healthcare reform beginning with tax year 2015, healthcare providers are required to provide Form 1095 to individuals who were eligible for or enrolled in healthcare coverage anytime during 2015. Form 1095 indicates the months of the year that you (and your dependents, if applicable) were offered or were enrolled in medical coverage during 2015.

You may need Form 1095 or the information it includes in order to file your federal tax return for 2015. Alcatel-Lucent cannot offer tax advice, so you might consider consulting a tax advisor for further guidance on Form 1095. (Please note: This form will not replace any state forms you may receive that provide proof of medical insurance.)

When and How You Should Expect to Receive the Form(s)

Forms for the 2015 tax year must be mailed to you no later than February 1, 2016. If you prefer that the Form 1095 be mailed to you, then no action is required.

However, you do have the option to be notified electronically — via email, text or both — when your Form 1095-C is available. If you elect electronic notification, you can access the form earlier than if you wait for it to arrive by mail.

Please note: If you sign up for electronic notifications, you will not receive your Form 1095-C via email. Instead, you will receive an email and/or text (based on your selected preference) informing you that the form is available online for downloading and/or printing.

How to Elect to Receive Electronic Notification

  • Log on to Your Benefits Resources at http://digital.alight.com/alcatel-lucent.
  • Click on the “Get Form 1095 Early” link.
  • Complete the fields under “Contact Information.”
  • Under “Notification Options,” indicate how you’d like to be notified — via email and/or text — when Form 1095-C is available.
  • Select “Save Notification Option Settings.”

When your request is processed, you’ll receive an electronic confirmation.
